IBA (Individual Bank Account)
Transfer Form
Please use use this form if you would like to transfer funds from your child's IBA to pay for allowable Music expenses.  The Gold mailbox at Lincoln-Way North (in the Music Department hallway) should be used to get your form delivered to us.
   

Travel Release Form When traveling outside of the Lincoln-Way North School District Parents or Legal Guardians are allowed to have their child(ren) return home from an event with them only if they present a valid travel release form to the appropriate chaperone once the band members are released by the Directors.
   

Medication Authorization Form When traveling overnight as part of a Lincoln-Way North group the use of medication needs to be specifically authorized.  Please use the form shown to the left to ensure that we have the proper documentation necessary so that medication can be administered from Chaperones to our students traveling as a group.
   
   
   
   

Spring Musical Ticket Order Form

Tickets will be sent by mail if order is received prior to April 15th.
Seating will be assigned as best available at the time your order is received.
Tickets are priced at $9 per ticket. Performance dates are Thursday, April 22nd, Friday, April 23rd, and Saturday, April 24th.

Cheesecake Order Form

Order a delicious Gourmet Delight, Inc. Cheesecake and turn it in (Gold Mailbox) no later than Monday, March 15th so that you can earn IBA money and eat good too!  Pick up will be the week of March 25th.  If there are any questions please contact Sue Ann Del Vicario at samd1989@sbcglobal.net or 815-351-7185.

Marching Phoenix Portrait
Order Form

With A Bit of Charm's help we have an opportunity to purchase individual portraits of our Marching Phoenix in full dress uniform during sitting sessions that are being offered the same night as our Sounds of the Stadium (Friday November 20th).  All Marching Phoenix are being encouraged to have their picture taken (no obligation to purchase) so that proofs can be viewed online at www.abitofcharm.com (by about Tuesday Nov 24th).  Decisions on orders can then be made at a later date based on the proofs that are displayed.

   

Madrigal Ticket Order Form

Ticket Reservations will be accepted beginning on October 1st, 2009. Tickets will be sold on a first come first served basis and they go very quickly!  There will be NO telephone or box office ticket sales.  Mail orders will only be accepted through Tuesday, November 25th.  After this date, any unsold tickets will be available through the box office on the night of the performance.  Once your order is filled, the tickets will be mailed to your home.  Please contact Mrs. Shoemaker if there are any questions (cshoemaker@lw210.org).

 

 

 

 

 

 

ISU Tailgate Ticket Order Form

Tickets for this event must be ordered in advance, we will not be able to serve anyone without a ticket. Band members are free. Chaperones must purchase a ticket.  Adults are $15.00, children under 10 yrs old are $8.00.

Please fill out the bottom with a check made out to the Music Boosters and put it in the gold mailbox marked Tail Gate no later than Saturday, October 17th.

   

Show Shirt Order Forms

These forms need to be filled out and placed into the gold mailbox by Friday September 25th.  Please contact Sue Ann Del Vicario or Sue Schlomas if there are any questions.

 

 

Alcohol Compliance Training

When volunteering for Levy corporate policies dictate that an alcohol compliance training session has to be completed before a volunteer is allowed to participate in an event..

 

 

Volunteer Application for
July 10th/11th
Paper work is needed in order to get things in place for our volunteer force.  Please fill out this application and return it to SueAnn Del Vicario either by fax or by dropping the completed forms into the Gold Mailbox.
